Low Testosterone and Hair Loss: The Paradox Most People Miss
The relationship between testosterone and hair loss is more paradoxical than the simple "more testosterone = more hair loss" assumption. Both high and low testosterone can affect hair — through different mechanisms. Here's the full picture.
01High DHT: the AGA mechanism
Most discussions of testosterone and hair loss focus on DHT — testosterone converted by 5-alpha-reductase at the scalp. High androgen sensitivity (not necessarily high testosterone levels) drives follicle miniaturization in the AGA pattern. Men with AGA often have normal testosterone levels; it's their follicles' sensitivity to DHT, not their total testosterone, that drives the loss.
02Low testosterone: a different problem
Paradoxically, very low testosterone in men can also cause hair changes — but diffuse thinning rather than patterned AGA. Testosterone supports the anagen phase generally; critically low levels can impair hair cycle maintenance throughout the scalp. This is more often a contributing factor than a primary cause, but it's worth ruling out in men with diffuse, non-patterned loss.
AGA follows the Norwood pattern — temple recession, crown thinning. Low-testosterone-related thinning tends to be diffuse across the whole scalp. If you have classic Norwood pattern, the testosterone level is probably a secondary variable. If the loss is diffuse, non-patterned, and you have other low-T symptoms, it's worth getting a full panel.
03TRT and hair loss
Testosterone replacement therapy raises testosterone — which can increase DHT conversion and accelerate AGA in men with genetic susceptibility. Men on TRT who notice hair loss should discuss concurrent finasteride with their prescribing physician. The two are compatible and frequently co-prescribed.
